Nice shirt!! I'm 5'10'', 175 lbs and a gym rat. My normal BB shirt size is 16 33. I got the Large, but in the Extra slim version and buddy, it is awesome! My first western shirt, won't be my last. January 26, 2012
Extra slim western Glad to report I I fit in this extra slim cut. With size inflation these days, it's nice to have a more tailored fit. I also have a regular cut of this shirt, which also works, but roomier. I generally dig western shirts, and love a BB take on it--especially appreciate the top snap instead of a single eccentric button at the collar. January 11, 2012
Chambray Shirt I am a very slim (skinny) person and got the X THIN Medium size shirt. It fits very well except in the neck. It appears that they have used the Normal Medium size neck instead of reducing the size. I would think that anybody who is 'Extra Thin' would also have a reduced neck size to go along with it. Otherwise, I really like the shirt. I will buy more. November 30, 2011

Answer: 
Large however with your build I would recommend going with a 16-34 slim fit shirt and avoiding the extra slim. If you do go with the extra slim I would add an inch onto your arms to account for the smaller yoke across the chest which brings the sleves up a tad.

Based on your measurements, a medium extra slim shirt should be the appropriate fit.
However, a large might work as well, if you want something a little loose.
